NYS Housing and Anti-Discrimination Disclosure

New York State law requires real estate brokers and salespersons to give this disclosure form (DOS-2156) to prospective buyers, sellers, tenants and landlords at the first substantive contact. It explains your rights under federal, state and local fair housing laws, gives examples of what real estate professionals may not do, such as steering and blockbusting, and tells you where to report discrimination.

You can read and download the current form below. The Department of State also publishes it in other languages, including Russian and Spanish, on dos.ny.gov.
